FHSAA Eligibility & Forms
Athletic Eligibility Information
Christopher Columbus High School is a member of the Florida High School Athletic Association (FHSAA) and the Greater Miami Athletic Conference (GMAC). Student-athletes must meet eligibility requirements set by both the FHSAA and Columbus. Incoming 9th grade students are eligible during their first semester if they were regularly promoted from 8th grade.
FHSAA Athletic Eligibility Requirements
The Florida High School Athletic Association (FHSAA) requires all student-athletes to complete their athletic eligibility and medical clearance electronically prior to participation in any sport. To be eligible, families must complete all required steps through the FHSAA-approved platform (Athletic Clearance). Student-athletes may not participate in workouts, tryouts, practices, or games until all forms have been submitted and approved.
Required Forms & Steps
All student-athletes must:
- Create an account and complete registration through the FHSAA-approved platform
- Upload the following required documents:
- FHSAA EL2 – Preparticipation Physical Evaluation (must be completed and signed by a licensed medical provider)
- FHSAA EL1 – ECG (Electrocardiogram) Screening Form
Beginning with the 2026–2027 school year, all first-time high school student-athletes are required to complete an ECG screening prior to participation. An ECG completed within the past two years satisfies this requirement.
If additional evaluation is needed following the ECG, clearance from a qualified medical professional is required before participation.
Additional Requirements
Student-athletes must also:
- Complete all required NFHS educational videos (Concussion, Sudden Cardiac Arrest, Heat Illness Prevention, and Sportsmanship)
- Meet all academic eligibility standards set by the school and FHSAA
- Submit any additional school-based forms as required
Important Notes
- All forms must be completed and approved before participation begins
- The EL2 physical must be fully completed, signed, and cleared without limitations by a healthcare provider
- These forms do not replace the Florida Certification of Immunization (DH 680) or School Entry Health Exam (DH 3040), which must still be submitted to the school
